How do you know when a campaign needs a new angle?

When a campaign gets some attention but does not turn into enough leads or sales, how do you decide whether to change the hook, the offer, or the audience? I am curious what signals people look at before rewriting everything.
 
I usually separate the signals before changing everything. If the ad gets weak click-through, I test the hook or creative first. If clicks are healthy but the landing page loses people, I look at message match and the offer. If both are reasonable but lead quality is poor, the audience is probably the issue. Changing one variable at a time for a fixed test window makes it much easier to see what actually improved performance.
